The approach we take to best meet the needs of our target population has three phases:
1. MEETING BASIC NEEDS
Upon entry to our residences Praxis Housing Initiatives provides basic concrete needs, food, change of clothing, and emergency medical care for all residents who require them. Clients are then assigned clean, safe, rooms. Once clients are settled, we conduct thorough interviews to assess individual needs. All new admissions are accessed for health care services, referrals are made to local community health care providers sensitive to our populations needs for optimum care. All new admissions receive a mental health evaluation by a NYC LCSW. Motivational interviews are conducted to access high risk behavior.
2. ONGOING SUPPORT
Our aim is to reduce recidivism to homelessness and increase housing sustainability with improvement in daily living skills and self-management, by working with clients to build an unbroken chain of support that begins at Praxis, one that acts as a buffer between them and the streets.
3. INTENSIVE INTERVENTION
Clients and case managers develop individualized service plans that help guide the clients towards self-reliance, and transitioning into independent living, leaving homeless in their past. Case workers and along with other supportive services staffs are assigned to meet with our clients weekly to help them reach the benchmarks set in the service plans.
What We Do
Praxis offers transitional housing, health and social services to homeless persons living with HIV/AIDS and other special needs. Our residents come with complex histories of physical and mental illness, Hepatitis C, TB, AIDS, addiction including formerly incarcerated. In addition to providing housing assistance, the following on site services are provided; vocational training, harm reduction skills, recovery readiness & LGBT support groups
