Title:Peer Specialist (outreach workers)

Job:

Canvas 10 block radius of assigned Praxis Housing Shelter, to intervene and address any public nuisance complaints with identified homeless person assigned to Praxis operated shelter. Discourage loitering, pan handling of homeless persons in the area. Contact DHS Mobile outreach units to report homeless persons in need of placement, medical and mental health assistance. Dispense hand out prepared meals onsite to residents residing in the shelter, keep record of meals handed out. Dispose of food container, keep refrigeration and microwave ovens clean, maintain common areas as needed. Meets with case management team, and provide input when needed; engages client assigned to the shelter and provide guidance and advice as needed. Engage with clients during meal times outreach door to door to encourage and promote onsite supportive groups and recreational activities. Promote smoke free, and drug free environment. Escort and assist clients to HRA, Social Security Administration and Housing interviews, as needed. Attends community resident meetings as assigned.

Qualifications:
H.S. diploma/GED and Certified Peer Specialist. Knowledge of DHS rules and regulations, and be able to maintain accurate and timely activities documentation. Bilingual (Spanish/English) preferred. Basic computer literacy required.

Experience:

 

  • 2-3 years of outreach work, peer educator.
Must have:
  • Food Handlers safety certification
  • CPR Training or willing to attain certification
  • Opiate Overdose training
  • Ability to administer Naloxone
  • Work alternate shifts and weekends
